Security Office or downloadable via AdZU website)

Photocopy of vehicle registration

Photocopy of enrollment form/official receipt
of student enrolled
2. FOR EXISTING GATE PASS HOLDERS (Renewal of Gate Pass)

Photocopy of enrollment form/official receipt of
student enrolled
B. For
Kreutz Campus (Tumaga)
A. FOR NEW APPLICANTS
a.
Gate Pass Application Form (available at the

1pc 2×2 ID picture of applicant (any
background) Photocopy of student identification card

Photocopy of vehicle registration

B. FOR EXISTING GATE PASS HOLDERS (Renewal of Gate Pass)

PROCEDURE
1.
Submit accomplished Application Form and other
requirements to the University Security Office
2.
Secure Payment Slip from USO.
3.
Proceed to the Finance Office for payment.
4.
Present official receipt to USO for validation.
5.
Claim Gate Pass sticker at USO.
Note:
Application for Gate Pass for 4-wheeled vehicles in Salvador
Campus is on hold until further notice.
