Protocol for Lost or Misplaced Student
ID Card
All
students are issued Identification Cards by the Office of Student Affairs (OSA)
for identification, protection and security purposes. ID card validation is done every Semester. (AdZU Student
Handbook 2.1)
A
student is required to wear/present his/her ID to the security guard on duty at
the gate before entering the campus. (AdZU Student Handbook 2.2)
No
student will be allowed to enter the campus for any purpose at any time on any
day, including Saturday afternoons, Sundays, and holidays, unless he/she
presents his/her id or an authorization approved by the OSA and University
Security Chief. (AdZU Student Handbook 2.3)
1. For
misplaced or forgotten ID, a student will only be allowed entry to the university
after proper screening has been done by the OSA
*For
habitual offenders (more than twice) a student will be given the corresponding
sanction (suspension)
*A
student is given a 24-hour (1day) grace period to present his/her ID
-The
OSA shall issue an EXCUSE SLIP (valid for 1 day), which also serves as a
temporary CAMPUS pass for the student.
-An ID is considered LOST if the
student fails to present it a day after the grace period.
2. For
lost ID, A student is required to secure an AFFIDAVIT of LOSS to be presented
to the OSA for the processing of the new ID. A Temporary ID shall be issued to
the student while the ID is being processed.
3. A
corresponding fee shall be charged from the student for the fabrication of a
new ID. Payment must be made through the
Finance office.
