April 8, 20, 26 May 4, 18, 2010 – Grades Four & Five
c. REQUIREMENTS:
1.AGE – Senior Preparatory – must be 5 years old by May 31, 2010
– Grade One – must be 6 years old by June 30, 2010
2.Clear photo copy of Birth Certificate – Present original copy
3.Photo copy of Report Card.
C. PROCEDURE:
1. Present the requirements to secure an application form from the Grade School Registrar’s Office.
2. Fill in the application form and pay the testing fee of P125.00 at the Finance Office. The testing fee is non-refundable.
3. Return the accomplished application form and the official receipt of the testing fee to the GS Registrar’s Office. The schedule for the test and the interview will then be set.
