INVITATION TO BID

All bids should be submitted in sealed envelope
to the Office of the Vice President for Administration on or before
the indicated deadline.

1.  Copy registration certificate/permit
 from Securities and Exchange Commission (SEC), Department of Trade and
Industry (DTI) for sole proprietorship, or Cooperative Development Authority
(CDA) for cooperatives

2.  Copy
of valid & current Mayor’s permit/registration issued by the city or
municipality where the principal place of business of the prospective bidder is
located.

3.   Copy
of BIR Tax Registration/Certificate of Owner/Business.

4.   List
of ongoing and completed government office / private company.

5.   Signed
Bid Prices in the Bill of Quantities

      6.   Signed
Detailed Estimates
