Graduate School Enrollment Process

ATTN | GRADUATE STUDENTS!

Check out the AdZU Graduate School Enrollment Process

STEP 1

NEW STUDENTS
• Fill out Registration Form: https://forms.gle/bK6CWEAGAyYde5cq7
• Fill out Student Waiver: https://host.adzu.edu.ph/revamp/graduate-school/
OLD STUDENTS
• Log-in to My AdZU Portal
• Accomplish Clearance and Teacher’s Evaluation (Second Sem, SY 2019-2020)

STEP 2

• Fill out Enrollment Form: https://bit.ly/AdZUGradSchEnrollment

STEP 3

• Wait for the assessed enrollment form to be sent to you via e-mail.

STEP 4

• Pay the School Fees (full or down payment) Available Payment Options:
1. Over-the-counter payment at the Finance Office
2. Online banking or over-the-counter payment through AdZU accredited banks.
3. Over-the-counter payment through accredited remittance Centers

STEP 5

• Scan or take a photo of deposit slip and email to aswigueedes@host.adzu.edu.ph/revamp or delacruzjudt@host.adzu.edu.ph/revamp.

STEP 6

Wait for the validated form to be sent back to you via email.

Note:
• Each teacher will be given an official list of enrollees.
• Your teachers will contact you using the contact details in your enrollment form before the start of classes.
