On-Line Enrollment Procedure 2012

ONLINE ENROLLMENT PROCEDURE 2012

FRESHMAN STUDENT

  1. Visit the Admission’s Office for instructions.
  2. Proceed to Finance Office or designated banks for payment.
  3. Visit the Office of Student Affairs for details regarding student ID.

REGULAR STUDENT

  1. Login at MyADZU (http://my.host.adzu.edu.ph/revamp) and click the online enrollment menu.
  2. Review subject(s) to be taken and follow the instructions at the portal.
  3. Payment through the ADZU Finance Office:
  • Present your ID number to the teller. There is no need to print your ONLINE ENROLLMENT SCHEDULE.
    Payment thru banks: (Bank of Commerce, Metrobank and China Bank)
  • Print your ONLINE ENROLLMENT SCHEDULE found at your ADZU Account and present it at ADZU designated banks.
  • Present Deposit Slip to the ADZU Finance Office. (Required)

    4. Proceed to Office of Student Affairs for ID validation.

IRREGULAR STUDENT

1. Please proceed to department chair for consultation. (Required)
2. If cleared on clearance, there are two choices:

  • Go to college lobby. Proceed to Step 3.
  • Login to MyADZU Portal and click the enrollment menu and follow the instructions at the portal. Proceed to Step 4.

3. Proceed to Student Activity Center (SAC) for encoding.
4. Payment through the ADZU Finance Office:

  • Present your ID number to the teller. There is no need to print your ONLINE ENROLLMENT SCHEDULE.

Payment thru banks: (Bank of Commerce, Metrobank and China Bank)

  • Print your ONLINE ENROLLMENT SCHEDULE found at your ADZU Account and present it at ADZU designated banks.
  • Present Deposit Slip to the ADZU Finance Office. (Required)

5. Proceed to Office of Student Affairs for ID validation.


NOTE:

  • Online Enrollment period is on June 2-7, 2012.
  • If payment is delayed (after June 7), student’s name will be removed from the subject slot.
  • Scholars proceed to Scholarships Office for other requirements before payment.
  • Nursing student can pay at least P 5,000.00 for down payment.
  • Non-Nursing student can pay at least P 2,000.00 for down payment.
  • No need for validation at the Registrar’s Office.
