Secure your slot at the earliest opportunity by confirming your admission with the College Admissions and Aid Office and submitting the following requirements:
- Original SHS report card
- Original Certificate of Good Moral Character
- College Entrance test result
- Photocopy of PSA certified birth certificate
- HBSAg (Hepa B) screening result
- Original chest X-ray result
- 1 pcs 1×1 colored picture
